American Specialty requires that a team or league have an already established system of securing waivers from all participants (adult or minor), in order to be eligible for insurance. Below, please find waivers recommended by American Specialty for your use. Please fill in the section defining the activity with the sport you bought insurance for, and secure signatures of all participants.

Please keep the signed waivers for your records. In the event of a claim, American Specialty will ask you to provide us with the waiver signed by the affected party.
